Unlike the 'king of nature', pinworms have neither a head (only the head end) nor a brain. All they can do is to cling to life with tremendous perseverance, using all the resources that allow maintaining the pinworm population even among the most civilized peoples (animals and birds do not get enterobiasis). Many have already guessed that today's conversation will focus on how to properly treat enterobiasis, that is, the invasion of pinworms.

This article is somewhat different from all similar drug reviews, since it is simply not enough to list even the most effective drugs for enterobiasis. Even if you fanatically follow the instructions, it is impossible to get rid of pinworms with just drugs alone. The reason for this is the phenomenon of human self-infection, the most powerful tool for pinworms in the struggle for survival. To properly recover from enterobiasis, you need to find out what self-infection is and what mechanisms it is implemented by.

Self-infection: a powerful survival mechanism

Those helminths that can infect one and the same host without needing other organisms for their development have a colossal advantage. After all, once having fallen into the final host, which releases eggs into the external environment, under favorable conditions they make it a lifelong source of eggs, in which more than one generation of worms lives, but more and more new ones – dozens of generations replacing each other.

Take opisthorchiasis, the causative agent of which is a feline fluke. Adult opisthorchis live in the liver, and they secrete eggs, which come out with human feces. But even if you constantly deal with the feces of a sick person, then it is theoretically impossible to become infected with opisthorchiasis. In order for the infection to occur, the eggs must enter a freshwater reservoir, go through a number of development stages. So, the intermediate host should be a fish of the cyprinid family with ripe opisthorchus metacercariae in their 'fish meat'. Only fish that is not well thermally processed is dangerous to humans. The eggs of the opisthorchus are completely safe for us, just like the eggs of bovine tapeworm, pork tapeworm and many other flat and tapeworms.

Pinworms are a completely different story. Pinworms lay their eggs in the area around the anus, and a person (most often a child) brushes this area at night, because he feels severe itching. Accordingly, microscopic pinworm eggs fall under his nails, and he calmly sends these infected hands together with toys into his mouth during the day.

Imagine that at this time, even the most modern anthelmintic drugs will be treated, which will destroy all pathogens in the intestines inside the human body. But if at least a few dozen eggs of pinworms remain under the nails, laid on the night before the treatment, and avoiding the destructive effect of the medicine, then they end up in the mouth, and self-infection will occur. In the end, though, the drug worked, but only on a part of the population, and therefore became useless.

Who are the pinworms?

Pinworms are small, and, in general, 'almost' harmless worms. They do not lead to serious and even fatal complications, such as roundworms or trichinella. Roundworms crawling from the esophagus into the larynx in a ball can simply suffocate a person during sleep, blocking the airways. Trichinella, hundreds and thousands of deposited in the muscles, cause toxic – anaphylactic shock. Pinworms are much smarter. An innate instinct, so to speak about creatures without a brain, tells them that the owner and the environment must be protected, and not taken to extremes. He must live as long as possible in order to constantly release eggs.

Therefore, enterobiasis is 'delicate'. This is the most common helminthic invasion in terms of the number of cases both in the Russian Federation and in countries around the world. When you hear the expression 'they found worms in our baby', then almost always, almost 100%, we are talking about pinworms. Pinworms, or enterobias, are dioecious parasites, and one end of them is slightly pointed, which is why they were called pinworms. Their body length is small, but females can be easily distinguished from males: females are up to a centimeter long, males are half as long.

Adult parasites attach to the intestinal wall and feed on its contents, but they can also feed on blood, but they themselves do not damage the intestinal wall, unlike hookworms, and do not cause anemia. Pinworms themselves live in the area of ​​the large intestine, and at the same time in adults – without symptoms, usually there is no pain. Of course, parasites can cause allergic symptoms, itching is pronounced, which especially worries babies.

As mentioned above, eggs are laid in the perianal folds, and there they develop, and after 6 hours, larvae appear from the eggs. They crawl back into the anus, enter the intestines, and develop into adult worms, the cycle repeats. At the same time, pinworm eggs are also actively swallowed, and they enter the intestines, thus, from all sides. In this case, self-infection also occurs from two sides.

Who can get enterobiasis: epidemiology

Most often, young children get sick and are able to maintain the persistence of pinworms in their bodies for a long time, especially when they are grouped together, for example, in a nursery and in the younger group of kindergarten. Children are not yet accustomed to the rules of personal hygiene, they often put their hands in their mouths, and this method of periodic self-infection makes it possible to expand the invasion to many babies.

The second important factor is that the eggs of pinworms are very viable, and they do not die right there, once in the external environment, but also retain the ability to invade for up to a month outside the organisms. By abundantly seeding children's toys, objects in the room, bedding, they pose a threat not only to children, but also to adults. Let's single out those risk groups of babies who are most often susceptible to helminthic invasions in general, and enterobiasis in particular, children:

  1. without hygiene skills that pull their hands into their mouths, mainly in younger age groups,
  2. from unsanitary and asocial families who live 'in mud';
  3. with poor health, often ill, who have low, including antihelminthic immunity;
  4. with delayed physical and mental development;
  5. who were artificially fed and did not receive real breast milk with protective factors – secretory immunoglobulins.

Finally, it should not be forgotten that 'Caesareans' are less adaptable than children 'received' as a result of physiological childbirth, as well as children whose mothers had aggravated factors of pregnancy.

By the way, enterobiasis is a strict anthroponosis, which means that only people get sick with it. No animals, snails, birds and animals are capable of being a source of infection. Of course, animals can be sources of very dangerous and even deadly helminthiasis, such as echinococcosis and especially alveococcosis. But as far as pinworms are concerned, everything is calm here. You don't have to worry that during the treatment the worms will 'crawl' onto the dog and cat, and then back onto the child.

Clinical picture

It was said above that pinworms do not cause any severe and acute abdominal pain, and in adults, the disease most often really goes away without symptoms. Indeed, in an adult, the immune defense is mature, and pinworms are still not as dangerous as some other types of helminths. And yet, for children, especially those with a weakened immune system, pinworms are dangerous, causing an allergic – toxic clinical picture, which consists of the following symptoms and signs:

  1. the child is worried about discomfort in the abdomen, near the navel, and weak, wandering, mild pain;
  2. there is an alternation of diarrhea, constipation and bloating – signs of intestinal dyspepsia associated with secondary dysbiosis;
  3. the symptoms of gastric dyspepsia are also disturbing. These are nausea and lack of appetite, belching and nausea, most often these signs are observed in a child in the morning;
  4. long-term enterobiasis causes increased fatigue and irritability. The child becomes whiny, due to pronounced itching in the anus, he does not sleep well;
  5. bruxism can be considered a sign of helminthiasis. This is the name of the creak of teeth in a dream, which occurs not only in a child, but also in adults, and is clearly audible at a distance;
  6. pinworms can not only crawl out of the anus onto the skin to lay eggs there, but also migrate a little further from the anus. Therefore, they can crawl into the genital tract of girls, and cause nonspecific microbial vulvovaginitis there, and if there are infections such as pyelonephritis or cystitis, then they are prone to periodic exacerbation against the background of enterobiasis;
  7. a long course of enterobiasis leads to a lag in physical development, the child loses weight;
  8. in the study of a general blood test, a characteristic symptom of helminthiasis is an increase in the number of eosinophils over 5% when calculating the leukoformula.

Of all this set of numerous symptoms and signs, one of the main ones that directly indicates pinworm infestation is severe itching in the anus at night. Why do pinworms lay eggs not in the intestines, but outside? Yes, simply because the developing larvae need air oxygen. Within a few hours, by the morning, the eggs mature, and after hatching of the larvae, potential infection is possible. If pinworms did not cause severe itching, then the child would not have insomnia, irritability and tearfulness.

But the harm of itchy skin is not only this. This is the most important factor in self-infection through the mouth and getting into other people, when the eggs of pinworms capable of development fall under the nails, and then into the mouth. In addition, a child with chronic microbial skin lesions, with impetigo, with streptoderma, can also bring in a secondary microbial infection when calculating.

Why do pinworms lay eggs and cause itching at night? Is there a kind of biological clock inside the organisms of worms? Naturally there are. At night, a person does not eat, and the concentration of nutrients in the intestines becomes different. But the most important thing is that the sphincter of the anus is able to relax at night, and it is much easier for a fertilized female to crawl out than during the day. The person is at rest, and nothing prevents pinworms from laying their eggs. The number of eggs laid by one female ranges from 10 to 15 thousand. The female lays eggs only once in her entire life, and then she dies.

Diagnosis of enterobiasis

Diagnosis of enterobiasis is very simple, and its reliability is close to 100%. Unlike opisthorchus, there are always a lot of pinworms, and they helpfully lay their eggs not inside, but outside. This compares favorably with opisthorchiasis. With opisthorchiasis, sometimes fecal analyzes are completely uninformative, it is necessary to repeatedly analyze the duodenal contents and probe, which, of course, exhausts the patient.

In order to diagnose enterobiasis, expensive blood tests are not needed at all, and no special financial costs. It is necessary to make a scraping from the perianal folds, and before carrying out the scraping, you should not wash yourself away, and you should not wash the child so as not to wash off these eggs. This scraping can be modified, and such an analysis is called an imprint of the perianal folds on adhesive tape, this is a common medical hypoallergenic tape.

It is relatively useless to look for pinworm eggs in feces, so donating feces is not recommended. After all, females attach eggs with special glue to the perianal folds so that they do not go anywhere, do not get lost, and do not come out with feces. Therefore, it is possible to donate feces for eggs to worms if you suspect pinworms, and it is useless, if eggs are found there, then in very small quantities, and outside the feces, having accidentally hit them. Such worms, paralyzed at the level of neuromuscular endings, easily leave the intestines. If it were not for the phenomenon of self-infection and the presence of eggs in the external environment, then there would be no problems with treatment at all. All existing remedies are very effective, well tolerated, and after they are taken, the parasites do not begin to actively leave the intestines and 'creep away, disturbed', this is very important. A person during treatment does not become more dangerous to those around him.

But in the event that the eggs do get into the environment (which happens almost always), then the danger of infection persists for up to 2 weeks. Therefore, an immutable rule for the treatment of enterobiasis is a repeated course of treatment after 2 or 3 weeks, and this is especially important when it is not possible to comply with all preventive and hygienic measures. Mebendazole is one of the main drugs for enterobiasis, it affects the metabolism of glucose in the tissues of parasites, depletes their energy supply. It is very convenient to dispense the suspension, since there is a measuring spoon and a special scale of divisions. This allows you to choose any amount of medication for the adult and for the child. Finally, the suspension can be prescribed for babies from the age of six months. However, despite the good tolerance, contraindications should also be considered. This is severe myasthenia gravis, pregnancy, caution should be exercised when treating patients with liver failure.
